== Diagnosis == To diagnose gynecomastia, a thorough history and physical examination are obtained by a physician. Important aspects of the physical examination include evaluation of the male breast tissue with palpation to evaluate for breast cancer and pseudogynecomastia (male breast tissue enlargement solely due to excess fatty tissue), evaluation of penile size and development, evaluation of testicular development and an assessment for masses that raise suspicion for testicular cancer, and proper development of secondary sex characteristics such as the amount and distribution of pubic and underarm hair. Gynecomastia usually presents with bilateral involvement of the breast tissue but may occur unilaterally as well. Diagnosis of men with breast enlargement can be evaluated using an algorithm. A review of the medications or substances an individual takes may reveal the cause of gynecomastia. Recommended laboratory investigations to find the underlying cause of gynecomastia include tests for aspartate transaminase and alanine transaminase to rule out liver disease, serum creatinine to determine if kidney damage is present, and thyroid-stimulating hormone levels to evaluate for hyperthyroidism. If these initial laboratory tests fail to uncover the cause of gynecomastia, then additional tests to evaluate for an underlying hormonal balance due to hypogonadism or a testicular tumor should be checked including total and free levels of testosterone, luteinizing hormone, follicle stimulating hormone, estradiol, serum beta human chorionic gonadotropin (β-hCG), and prolactin.
== Cause == CREST syndrome involves the production of autoimmune anti-nuclear (ANA) and anti-centromere antibodies (ACA), though their cause is not currently understood. There is no known infectious cause.
== History and evaluation == Krebiozen was initially promoted by Stevan Durovic, a Yugoslavian physician who claimed that the substance was isolated from the blood serum of horses inoculated with Actinomyces bovis. Durovic claimed that Krebiozen had been useful in the treatment of cancer, mainly in cats and dogs. His claims were backed by Andrew Conway Ivy, a prominent physiologist, and by several politicians including Senator Paul Douglas (D-IL). Ivy became convinced that Krebiozen possessed anti-cancer properties, administered the substance to himself and colleagues, then to a dog, and eventually started testing Krebiozen on patients. Ivy called a press conference in 1951 at which he announced to an audience of journalists, politicians, doctors and potential investors that Krebiozen was a success. At the press conference, Ivy claimed that of 22 treated patients, 14 were alive and none had died of cancer. However, in reality 10 of the treated patients had died—all of cancer—at the time of the press conference. Shortly afterward, the Krebiozen Research Foundation was established by the Durovic brothers, with Ivy as president. Intrigued by Ivy's announcement, 10 hospitals and cancer research centers followed up on the trial and attempted to reproduce Ivy's claimed results. None of these independent researchers observed any effect of Krebiozen on cancer. A compilation of these institutions' negative data was reported in the Journal of the American Medical Association in 1951. Krebiozen's backers responded by alleging a conspiracy against the drug.
